To receive the school immunizations, please bring all of the following information to the Health Department with you:
- Up-to-Date Immunization Record
- Insurance Cards
- Parent or Guardian must be present
Cost?
If uninsured: No charge for vaccine. Administration fee slides based on income. Charge ranges from no charge to $20 for each immunization. Physicals are on a sliding-fee scale.
Tenncare or Private Insurance: Health Department will file insurance for payment.
